Было проведено интенсивное изучение биомаркеров, поскольку их потенциал в качестве диагностических инструментов в случаях mTBI позволит обеспечить точную диагностику, улучшить скорость ведения пациентов и снизить медицинские расходы.

Падение во время катания на лыжах, падение на лестнице или удар по голове могут вызвать такие симптомы, как помутнение зрения, рвота, потеря сознания или памяти в течение примерно 30 минут. В таких случаях существует риск легкой травмы головного мозга, которая составляет более 90% повреждений головного мозга, из-за которых людей направляют в больницы. На сегодняшний день единственным средством для постановки надежного диагноза является компьютерная томография (КТ), которая доступна не в каждой больнице, при этом пациент подвергается лучевой нагрузке.

Международная группа ученых, сотрудничающих с Женевским университетом (University of Geneva; Женева, Швейцария), рассмотрела пациентов из трех разных европейских городов: Женевы, Севильи и Барселоны. В когорту 1 были включены 132 пациента, из которых 21 были КТ-положительными (16%). Независимая когорта 2 использовалась для проверки панели на 109 пациентах, из которых 17 человек (16%) были КТ-положительными.

Команда индивидуально оценила 13 белков на их способность быть различимыми между пациентами с поражением головного мозга или без в соответствии с результатами КТ. Команда выделила четыре молекулы, свидетельствующие о наличии черепно-мозговой травмы: сердечная форма белка, связывающего жирные кислоты (H-FABP), интерлейкин-10, кальций-связывающий белок B (S100B) и глиофибриллярный кислый белок (GFAP). Было отмечено, что уровень H-FABP в отдельности позволяет подтвердить отсутствие риска травмы у одной трети пациентов, госпитализированных после шока.

Команда разработала небольшое устройство - портативный тест, который анализирует уровень белов в крови и позволяет, используя одну каплю крови, диагностировать наличие легкой черепно-мозговой травмы. Быстрый портативный диагностический тест под названием TBIcheck был основан на принципе тестирования на беременность: помещая одну каплю крови в лунке небольшого пластикового корпуса размером 5 см, пациент в течение 10 минут получает информацию, есть ли риск легкой травмы, а именно, является ли его уровень H-FABP выше 2,5 нг/мл крови. В случае сомнений при чтении результата на TBIcheck может быть установлен небольшой считыватель Cube Reader. Он отобразит слово "положительный" или "отрицательный" и отправит результат на смартфон пациента или лица, оказывающего ему помощь, через Bluetooth. Тест TBIcheck будет выпускаться компанией Advanced Brain Companion Diagnostics (ABCDx, Женева, Швейцария), в продаже он появится в 2019 году.

Доктор философии Жан-Чарльз Санчес (Jean-Charles Sanchez), профессор и старший автор исследования, сказал: "Наше исследование показывает, что результаты становятся еще более точными, когда мы объединяем уровни H-FABP и GFAP.
