外周血白细胞的分类计数是临床医师送检最频繁的化验之一,因为它是诊断各类血液病和全身疾病的一项重要检验。化验技术的改进得益于自动化血液细胞计数器的开发,自动血液计数器已逐渐取代仍在大多数实验室实行的人工显微镜下计数。

韩国首尔蔚山大学医学院与牙山医学中心的实验科学家检查了308份有定量异常或定性异常的外周血样本,临床医师此前已经要求做这批样本的全血计数。先用日本神户Sysmex公司的XE-2100自动血细胞分析仪分析这些样本,再确定白细胞的总计数和分类计数。让两名受过良好训练的实验室技师分别在显微镜玻片上对100个细胞进行人工分类计数。

给玻片做标记,进行人工显微镜下分类计数后装入瑞典隆德CellaVision公司的CellaVision DM96系统。制备血沉棕黄层时,制备前用样本制备玻片,在CellaVision DM96系统内做标记。对于白细胞数目少于1,000细胞/微升的样本,CellaVision DM96系统将细胞计数增加到300个或500个细胞,为了评估这一功能的临床相关性,研究人员对CellaVision DM96系统和人工计数进行了相关性分析。

两种方法的相关系数很高且一致性很好,范围是0.864至0.992。这套系统识别异常的灵敏度、特异性、阳性预测值、阴性预测值都很高,特别是对母细胞。当操作员给仪器下指令计数300或500个细胞时,其效果优于100个细胞,而时间平均只牺牲了40秒/玻片。

该研究的论文发表于2013年10月号的《国际血液化验杂志》(International Journal of Laboratory Hematology)。作者总结说,临床实验室可以用CellaVision DM96系统与人工计数的准确度进行比对。在白细胞减少的样本中,操作员要求计数300或500个细胞可提高结果质量,同时不会严重延长运行时间。
