血浆中特定的小分子也许可用于确定一个人是否有持续的轻度创伤性脑损伤(mTBI),俗称脑震荡。

一组科学家与美国加利福尼亚大学尔湾分校(https://uci.edu)的同事合作,从2009至2014年间列入冲撞性运动名单的632名运动员中选部分人抽取血样。参与者的年龄、性别和运动情况与充当对照者(NC)的队友相仿。诊断出mTBI的运动员受伤后6小时内接受静脉切开术,然后依次在受伤2天(2d)、3天(3d)和7天(7d)后与作为对照的对等NC队友一道接受静脉切开术。
 
在两天分两批执行代谢组学分析,但使用同样的液相色谱–质谱(LC-MS)设备。简言之,按照以前公布的预案,依次提取所有血浆样本未被靶向的代谢组学特征。一旦预先标注了假定的代谢组生物标志物,用串联质谱(MS/MS)测定从原始的生物标志物探究队列中随机选取的病例样本和对照样本,根据现有标准或合成标准,或者确证这些生物标志物,或者拒绝它们。通过MS/MS谱匹配确认的代谢物被认为以高置信度得到完全确证。
 
科学家发现了一组大学生运动员血浆里的六个特定的小分子,已诊断出这些运动员有脑震荡。当评估这些分子时,它们的同时出现能准确预测队友是否有持续的mTBI。这表明这些小分子也许是mTBI的临床相关的生物标志物,可能还有其它分子。然后检验另一组受试者的同六个生物标志物,有的人有mTBI,有的人没有,结果与运动员的检查结果一致。
 
该研究的论文发表于2018年4月20日发表于《公共科学图书馆》杂志《ONE》分册。论文合著者之一、退休的神经外科医生Massimo S. Fiandaca大夫说:“这样的血检对于确定某人是否有持续脑震荡很重要,目前这不是一项轻松的任务,但也许事实最终将证明它可用于确定伤员何时可以安全地恢复日常活动。”
