2型糖尿病(T2DM)是一种多因素诱发的代谢病,其特征是高血糖,起因是胰腺β细胞分泌胰岛素的功能受损以及细胞对胰岛素反应不力。

在糖尿病前期,一旦葡萄糖水平升高,就会出现两个特征中的至少一个:空腹葡萄糖(IFG)水平下降与葡萄糖耐受性(IGT)下降。IFG和IGT在代谢意义上不等价,它们很可能反映了导致T2DM的不同病理生理状态。

丹麦哥本哈根斯丹诺糖尿病中心的科学家及其同行对明确表现型男性(107例病例,216例对照)的血浆样本进行全脂质组测定,这些男性参与了基线水平与五年随访期的纵向研究。为了确证脂质标志物,科学家们又开展了一项研究,对象是631名成年男子的典型样本。

用基于高效液相色谱加飞行时间质谱的脂质组学平台共分析了277份血浆脂质。然后在美国马萨诸塞州米尔福德市Waters股份有限公司的Q-Tof高级质谱仪与Acquity高效液相色谱仪(UPLC)上分析提取出的脂质。通过计算选出能最有力预测T2DM发生的脂质并加以确证,再与无脂质的标准风险模型比较。根据OGTT和HbA1c测量得到的血浆葡萄糖水平评估葡萄糖耐受状态。

研究人员发现,三酰甘油和二亚酰磷脂水平升高以及烷酰基磷脂酰胆碱水平降低时,在诱发T2DM的因子里观察到恒定的脂质特征。溶血磷脂酰胆碱酰基C18:2 (LysoPC(18:2))、磷脂酰胆碱PC(32:1)、PC(34:2e)和PC(36:1)以及三酰甘油TG(17:1/18:1/18:2)被选为包括代谢风险因子的完整模型。当进一步根据体重指数(BMI)和年龄做调整后,这些脂质预测T2DM进展的风险比分别为0.32、2.4、0.50、2.2和0.31。

该研究的论文发表于2018年1月号的《代谢临床与实验》(Metabolism Clinical and Experimental)杂志。作者总结说,他们的研究说明,T2DM特有的脂质特征在确诊的数年前就存在,它能更好地预测向T2DM的转化。对高风险组的研究也显示分子脂质生物标志物有预测能力,而这种情况下标准风险因子无助于区分致病因子和非致病因子。
