Большинство случаев этого заболевания обусловлено пожилым возрастом и образом жизни и лишь некоторое количество случаев связано с генетическими нарушениями.

Диагностика колоректального рака выполняется путем взятия проб участков толстой кишки, подозрительных на предмет возможного развития опухоли, обычно это производится во время процедуры колоноскопии или ректороманоскопии в зависимости от места поражения. Диагноз подтверждается посредством микроскопического исследования образца ткани. В Европе пятилетняя выживаемость при колоректальном раке составляет менее 60%, а в развитых странах умирает около трети людей, заболевших этой болезнью.

Международная группа ученых, возглавляемая коллегами на медицинском факультете Университета имени Джонса Хопкинса (Балтимор, штат Мэриленд, США), включила в исследование 58 пациентов из четырех шведских больниц с КРР I, II или III стадии, которым сделали радикальную хирургическую резекцию в период с 2 февраля 2007 г по 8 мая 2013 г. Образцы крови были взяты через месяц после хирургической процедуры, затем их брали каждые 3–6 месяцев для анализа циркулирующей опухолевой ДНК (цоДНК). Пациенты наблюдались до тех пор, пока не обнаруживались метахронные метастазы, или в среднем до 49 месяцев. Анализ данных проводился с 1 марта 2009 года по 23 июня 2018 года.

Хотя большинство пациентов не имели выявляемых уровней цоДНК в крови после операции, поиск известных опухолевых мутаций у пациентов с использованием анализа Safe-SeqS позволил выявить более дюжины пациентов, положительных к цоДНК. Исследователи обнаружили, что возврат и рецидив заболевания произошли у всех пациентов, кроме 3 из 13 пациентов с ранней стадией КРР с послеоперационной цоДНК, что указывает на потенциальную пользу продолжающегося тестирования цоДНК после хирургической резекции опухоли. В среднем цоДНК обнаруживалась у рецидивирующих пациентов примерно за три месяца до того, как клиницисты смогли увидеть рецидив заболевания с помощью рентгенологического скрининга или клинических симптомов. Команда не наблюдала рецидив заболевания в течение более чем четырехлетнего периода наблюдения у 45 оставшихся без цоДНК пациентов с КРР.
