Интересные результаты, полученные в недавно проведенном пилотном исследовании, продемонстрировали возможность использования молекулярных опухолевых маркеров в качестве основы для выбора химиотерапевтических препаратов для применения у пациентов с метастатическим раком поджелудочной железы.

Исследователи из Джорджтаунского университета (Вашингтон, округ Колумбия, США) и сотрудники нескольких других учреждений провели исследование, предназначенное для оценки возможности применения алгоритма лечения, основанного на интерпретации трех известных прогностических маркеров ответа на химиотерапию: каталитической субъединицы рибонуклеотидредуктазы M1 (для гемцитабина), гена кросс-комплементирующей эксцизионной репарации (для препаратов на основе платины) и тимидилатсинтазы (для 5-фторурацила) у пациентов с нелеченным метастатическим раком поджелудочной железы.

Однако 10 пациентов не прошли скрининг, в первую очередь из-за недостаточной доступности к опухолевой ткани. У оставшихся 20 пациентов был взят анализ биопсии опухоли для назначения одной из семи схем двухкомпонентного лечения. Ключевые второстепенные цели включали частоту ответов (ЧО), частоту контроля заболеваний (ЧКЗ), выживаемость без прогрессирования заболевания (ВПЗ) и общую выживаемость (ОВ).

Результаты продемонстрировали многообещающие показания для выживаемости без прогрессирования и общей выживаемости с частичным ответом, наблюдаемым у 28% пациентов, и стабилизацией заболевания у 50% по завершении исследования.

На основании результатов данного пилотного исследования была начата вторая стадия более масштабного рандомизированного исследования молекулярно-направленной терапии в сравнении с выбранным врачами стандартом медицинской помощи.

Результаты пилотного исследования прошли обсуждение в интернет-издании журнала Journal of Pancreatic Cancer от 2 мая 2019 года.
