Однако процесс обследования в отделениях неотложной помощи (ОНП) пациентов с подозрением на острый инфаркт миокарда (ОИМ) от проведения лабораторных испытаний до начала терапии отнимает много времени.

Диагностические тесты в пунктах медицинской помощи, которые в большинстве случаев предоставляют результаты в течение нескольких минут, используются во всем мире, поскольку больницы испытывают повышенную нагрузку из-за переполнения отделений неотложной помощи и более длительного времени ожидания. Цель диагностических тестов, проводимых в присутствии пациента - получить результаты с точностью, аналогичной золотому стандарту высокочувствительных лабораторных тестов.

Команда ученых, сотрудничающих с Госпиталем Крайстчерча (Крайстчерч, Новая Зеландия), провела оценку состояния 354 взрослых, из которых 255 (72,0%) были мужчинами; средний возраст участников составлял 62±12 лет, поступившие в отделение неотложной помощи имели симптомы острого коронарного синдрома, например сердечный приступ. Концентрации тропонина измерялась при поступлении в ОНП с использованием как нового метода анализа i-STAT TnI-Nx, так и высокочувствительного анализа на тропонин I, Architect hs-cTnI.

Исследователи сообщили, что из 354 пациентов 57 (16,1%) имели ОИМ. Восемьдесят пять пациентов (24,0%) были доставлены в ОНП менее чем через три часа после начала симптомов. Не было обнаружено различий между площадью под кривой (ППК) в случае анализа TnI-Nx (0,975) и анализа hs-cTnI (0,970). Результат анализа TnI-Nx менее 11 нг/л выявил 201 пациента (56,7%) как имеющих низкий риск, со 100% чувствительностью и прогностическим значением отрицательного результата. Для сравнения результат анализа hs-cTnI менее 3 нг/л выявил 154 пациента (43,5%), имеющих низкий риск, с чувствительностью и прогностическим значением отрицательного результата 100%.

Авторы пришли к выводу, что новый анализ на тропонин для использования в пунктах оказания помощи, который может дать результат через 15 минут после взятия крови, имел сопоставимую с анализом hs-cTnI распознавательную способность для исключения ОИМ после единственного анализа крови. Использование в ОНП может способствовать более раннему принятию решений и позволит ускорить безопасное отделение значительной части пациентов с низким уровнем риска. Дополнительная польза от этого анализа, по всей видимости, будет выявлена за пределами ОНП, например в сельских больницах и в учреждениях общего профиля, где существует ограниченный доступ к лабораторным анализам тропонина.
