Ожидается, что в 2019 году в США от метастатической меланомы могут умереть более 7200 человек, причем мутации BRAF играют роль почти в половине таких диагнозов.

В недавнем исследовании обращалось внимание на преимущество природы раковых клеток, которые непрерывно умирают и заменяются новыми клетками, что является частью агрессивного роста раковых опухолей. Опухолевые клетки, умирая, разрываются, высвобождая свою ДНК в кровь, где их можно измерить с помощью анализов, что позволяет улучшить диагностику и более целенаправленно проводить лечение, основываясь на опухолевой ДНК каждого пациента.

Команда ученых, возглавляемая сотрудниками Медицинской школы Нью-Йоркского университета (NYU School of Medicine, Нью-Йорк, штат Нью-Йорк, США), проанализировала образцы крови от 345 пациентов мужского и женского пола, у которых были мутации BRAF с меланомой III или IV стадии, уже распространившейся из кожи в другие органы. Эти пациенты не могли получать хирургическое лечение и были частью большой группы пациентов, участвующих в клиническом испытании препаратов дабрафениб и траметиниб, предназначенных для лечения злокачественных опухолей с BRAF. Анализ на циркулирующую опухолевую ДНК (цоДНК) был разработан Bio-Rad Laboratories.

Среди основных результатов исследования было обозначено, что мутация опухоли BRAF могла быть обнаружена с помощью нового анализа крови у 93% пациентов до начала лечения. Кроме того, команда обнаружила, что цоДНК BRAF больше не регистрировались после одного месяца терапии у 40% пациентов, у которых был положительный клинический результат после целевой терапии, что оценивалось по среднему временем выживания 28 месяцев. Напротив, у 60% пациентов, у которых не было ответа на лечение, все еще наблюдались обнаруживаемые уровни цоДНК и выживаемость составляла в среднем всего 14 месяцев.

Дэвид Полски (David Polsky), доктор медицины, профессор дерматологии и старший исследователь, говорит: “Наше исследование предоставляет убедительные доказательства того, что отслеживание этой генетической информации может быть полезным при выявлении пациентов, у которых раковые опухоли уменьшаются и которые дольше выживают в результате применения определенной схемы лечения. Если дальнейшее тестирование окажется успешным, мониторинг образцов крови на BRAF может дать нам раннее указание на то, нужно ли нам корректировать план лечения пациента”. Исследование было представлено на ежегодном собрании Американского общества клинической онкологии, которое проходила 31 мая–4 июня 2019 года в Чикаго, штат Иллинойс, США.
