Синдром поликистозных яичников (СПКЯ) диагностируется по наличию двух различных наборов диагностических критериев, что приводит к фенотипическому спектру случаев СПКЯ. Генетическое сходство между случаями, диагностированными на основе этих двух критериев, в большинстве случаев не было выявлено.
Международная группа ученых, в сотрудничестве с Университетом Юты (Солт-Лейк-Сити, штат Юта), провела метаанализ общего ассоциативного исследования с участием 10 074 человек с СПКЯ и 103 164 без него, в результате чего были выявлены 3 ранее не обнаруженных локуса и 11 локусов, связанных с нарушениями работы эндокринной системы в прошлом. Из них 13 локусов были связаны с самостоятельно заявленными СПКЯ и случаями заболевания, диагностированными с использованием критериев Национального института здравоохранения (НИЗ) или критериями, предложенными в Роттердаме.

Как объяснила команда ученых, диагнозы СПКЯ основаны на выявлении повышенного уровня андрогенных гормонов и нарушений овуляции, что затрагивает примерно 7% женщин репродуктивного возраста во всем мире. С другой стороны, более общие критерии, выработанные в Роттердаме, определяли СПКЯ как морфологию поликистозных яичников в сочетании с гиперандрогенизмом или нарушением овуляции, что затрагивает примерно от 15% до 20% женщин во всем мире.

Команда сообщила о новых связях в локусах генов PLGRKT, ZBTB16 и MAPRE1 или рядом с ними, которые, как полагается, способствуют метаболическим и репродуктивным путям. Метаанализ также привел к 11 известным локусам риска, которые были представлены в чрезвычайно большом количестве для вариантов в генах и вблизи из нейроэндокринных и метаболических путей. Набор известных локусов риска включал в себя полдюжины участков, ранее причастных к СПКЯ у женщин, имеющих китайские корни, и вариант вблизи генов GATA4/NEIL2, который показал сильную связь с СПКЯ, диагностированным с использованием критериев НИЗ, но более слабую связь с самостоятельно заявленными случаями СПКЯ.

Авторы пришли к выводу, что генетические основы СПКЯ включают нейроэндокринные, метаболические и репродуктивные пути в патогенезе заболевания. Хотя необходимы специфические стратифицированные анализы фенотипа, генетические результаты были согласованы по диагностическим критериям для всех, кроме одного локуса восприимчивости, что указывает на общую генетическую архитектуру, лежащую в основе различных фенотипов.
