MALDI-TOF MS系统发现了有重要临床意义的酵母菌

研究人员评估了两套基质辅助激光解吸/电离飞行时间质谱系统(MALDI-TOF MS)鉴定有重要临床意义的酵母菌分离物的性能。

快速鉴定酵母菌病原体有助于及时开始有效的抗真菌疗法,并能缩小疗法的选择范围,避免用有毒的抗真菌药治疗,改进治疗效果,降低成本。

科威特大学(Safat, Kuwait; www.kuniv.edu)的微生物学家收集了188份临床相关的真菌分离物,这些分离物来自血液培养物、支气管肺泡灌洗、脑脊液、尿液、伤口以及上阴道拭子和宫颈内拭子。

先用生物梅里埃公司(Marcy l’Etoile, France)的VITEK 2系统鉴定临床酵母菌分离物。必要时还要执行一种或多种检验,在沙保氏葡萄糖琼脂(SDA)上进行形态学检验,对念珠酵母菌进行芽管形成试验,对隐球酵母进行尿素酶同化试验,这些检验使用碧迪公司(Sparks, MD, USA)的产品。从分离物中提取蛋白质,在Bruker Daltonics公司(Bremen, Germany)的MALDI-TOF Bruker Biotyper MS和生物梅里埃公司的MALDI-TOF VITEK MS上分析。

VITEK 2的识别准确率为94.1% (177/188),VITEK MS的准确率为93.0% (175/188),Bruker Biotyper MS的准确率为92.6% (174/188)。VITEK MS未能鉴定出三份分离物,九份拟平滑念珠菌被误认为近平滑念珠菌,因为数据库中没有该菌种。Bruker Biotyper未能识别或误识11份分离物,另外14份得到正确识别。

该研究的论文发表于2014年9月号的《国际传染病杂志》(International Journal of Infectious Diseases)。作者总结说,MALDI-TOF MS方法为鉴定临床样本中的酵母菌提供了一套标准化的工作方案。它运行时间短,数据库可扩充,说明它适合作为常规临床微生物实验室鉴定酵母菌的一线检验。
