BRAF突变检测成为新型PCR分子诊断系统的关键

比利时梅赫伦的Biocartis公司最近发布的Idylla仪器是该公司旗舰分子诊断系统的主打产品。发布Idylla的同时还发布了Idylla BRAF突变检验,用以检测BRAF致癌基因的突变。

BRAF(v-Raf小鼠肉瘤病毒致癌基因同源物B1)基因编码的蛋白质是丝氨酸/苏氨酸蛋白激酶B-Raf。约半数黑色素瘤表达BRAFV600E突变(在B-Raf蛋白上氨基酸位置号600处,正常的缬氨酸被谷氨酸替代)。从2011年8月起,威罗菲尼(vemurafenib)等B-Raf抑制剂获准用于治疗转移性黑色素瘤。除黑色素瘤外,还经常在乳头状甲状腺癌和大肠癌病例中观察到这一突变。

预计Idylla系统将树立分子诊断检验的新标准。它的按需控制设计使临床医师能够在指定的任何时间开始一项新检验,无需排队或受过培训的操作员。该系统运行时间短,在35分钟和120分钟之间,因而化验室能在当天报告结果,大大加速了治疗决策过程。Idylla采用不透液体的可抛型样本盒,降低了污染风险,无需分别进行PCR前和PCR后实验。

BRAF突变检验是Idylla的第一项肿瘤检验,其设计目的是为特定类型的皮肤癌确定最恰当的治疗。该测定能直接检测福尔马林固定石蜡包埋(FFPE)组织切片的突变,需时约90分钟。对FFPE组织切片的直接处理据称为BRAF检验所特有。

Biocartis公司CEO Rudi Pauwels说:“分子检验在治疗决策过程中发挥着关键作用。它使医生可以根据病人的基因特征选择特别设计的疗法。以前,分子诊断要求一系列耗工耗时的专门步骤。结果,大多数实验室自己不能执行这些检验,而是把它们送到专业实验室,在那里编入队列,常常延误治疗决策。我们相信Idylla将完全颠覆分子实验室的日常业务,使分子检验变得不可思议地简单。这将极大加速诊断过程,并将使医生能够在短得多的时间内做出治疗决策。最终,病人将更快接受到个性化治疗,得到更好的治疗效果。”
