Point-of-Care Molecular Technology Promises Transformative Shift in Oncology

Prolonged waits may also heighten anxiety and increase the risk of missed follow-ups, further complicating care pathways. Point-of-care (POC) molecular platforms are designed to deliver actionable results near the site of care through faster, simpler workflows and lower-cost testing.

At the University of Illinois Urbana-Champaign’s Cancer Center at Illinois (CCIL), an interdisciplinary team coordinated a multi-institutional review of point-of-care (POC) molecular diagnostic technology. The article examines how POC testing could influence cancer detection, therapy selection, treatment-response monitoring, and personalized medicine. Collaborating institutions included the University of Illinois Chicago, Huntsman Cancer Institute, Mayo Clinic, and Stanford School of Medicine, and the work was published in Lab on a Chip.

The review outlines how POC systems combine advanced biosensing platforms capable of “digital precision” detection and counting of individual molecules and viruses with novel biochemical methods that simplify and accelerate workflows. It highlights a liquid biopsy POC technology developed at CCIL that can deliver oral cancer results in less than 15 minutes. The authors also emphasize the importance of close clinician–engineer collaboration to demonstrate broadly applicable use cases.

Drawing on clinical scenarios and case studies spanning oral squamous cell carcinoma, hepatocellular carcinoma, prostate cancer, and non-small cell lung cancer, the review presents both opportunities and obstacles for POC cancer diagnostics. Challenges include funding and regulatory approval, variations in sample-collection methodologies, and the realities of manufacturing and commercialization. The authors note that continued dependence on centralized laboratory testing remains a major obstacle in cancer detection, therapy selection, and treatment monitoring.

Beyond clinical examples, the review examines the global funding landscape, intellectual property requirements, and regulatory frameworks that influence the transition of POC cancer diagnostics from academic research to commercial use. The authors position the work as a roadmap for integrating these technologies into routine clinical practice. They also highlight societal, personal, and financial barriers—including result-related anxiety, missed follow-up appointments, and limited access in rural areas—that could be partly addressed through low-cost, rapid, noninvasive tests performed and interpreted during a single clinic visit.

“Today, many molecular diagnostic tests still rely on centralized labs and delayed turnaround times, whereas the vision outlined in this work is a future where clinically actionable molecular information can be obtained much closer to the POC, enabling faster treatment and management decisions,” said Seemesh Bhaskar, first author of the team’s paper.

“The burden of cancer urges us to develop swift, precise and accessible diagnostic responses that can overcome the limitations of diagnostic tests performed in centralized laboratory settings. Point-of-care molecular diagnostics are emerging as transformative tools that enable decentralized testing across hospitals, outpatient clinics, oncology practices and dental practices, thereby accelerating clinical decision-making to improve patient outcomes,” said Brian Cunningham, program leader at the Cancer Center at Illinois.
