Помимо артефактов, которые возникают при взятии и транспортировке крови, используемый антикоагулянт in vitro потенциально влияет на результат теста свертывания крови.

Обычно соотношение крови к цитрату натрия 9:1 используется с буферным диапазоном от 3,2 до 3,8%. 3,2%-ный буферный раствор цитрата натрия связывает меньше добавленного в анализ кальция, чем 3,8%-ный буферный раствор цитрата натрия, поэтому время свертывания, как правило, у 3,2%-ого буферного раствора цитрата натрия короче, чем у 3,8%-ого. Точные данные отсутствуют, но в настоящее время широко используется 3,2%-ный буферный раствор цитрата натрия.

Ученые из Медицинской лаборатории Венского медицинского университета (Medical University of Vienna; Вена, Австрия) провели проспективное обсервационное исследование с участием 76 добровольцев. Были оценены различия, связанные с концентрацией цитрата натрия. Для обеих концентраций буферного раствора были установлены референтные диапазоны. При использовании аналогичных калибровочных параметров были последовательно проанализированы образцы 3,2%-ого и 3,8%-ого буферных растворов цитрата натрия. Комбинации реагентов-анализаторов были выбраны с точки зрения точности и достоверности измерений, надежности анализов и практичности на этапе ввода параметров в лаборатории.

Чувствительный к волчанке реагент активированного частичного тромбопластинового времени (activated partial thromboplastin time lupus-sensitive reagent - aPTT-LA) для контрольного анализа волчаночного антикоагулянта (lupus anticoagulant - LAC) был проанализирован с использованием плазмы с реагентом, содержащим фосфатидилэтаноламин, или без него. Контрольный анализ волчаночного антикоагулянта считался положительным, когда различие между временем свертывания aPTT-LA было больше восьми секунд или соотношение между чистым временем свертывания под воздействием разбавленного яда гадюки Рассела (diluted Russell viper venom time - dRVVT) и dRVVT Confirm (dRVVT Ratio, Staclot DRVV Screen/Staclot DRVV Confirm) было больше 1,25.

Команда обнаружила, что большинство оцениваемых параметров показало хорошую сопоставимость между цитратными образцами, взятыми с 3,2%-м и 3,8%-м буферным раствором тринатриевого цитрата. Эллагиновая кислота, содержащая реагент активированного частичного тромбопластинового времени (aPTT-FS), указывала системную и пропорциональную разницу между обеими концентрациями буферных растворов, что приводило к изменению его референтных диапазонов. Кроме того, контрольный анализ на волчаночный антикоагулянт (Staclot LA) показал лишь умеренную корреляцию с пропорциональным отклонением между концентрациями цитрата. Статистически значимое различие было обнаружено в контрольном анализе с разбавленным ядом гадюки Рассела, факторах свертывания V и VIII, а также незначительная клиническая значимость была обнаружена в активности протеина С.

Авторы пришли к выводу, что изменение 3,2%-ого и 3,8%-ого буферных растворов цитрата натрия в специализированных тестах на коагуляцию в данных условиях имело ограниченную значимость. В качестве исключения концентрация цитрата оказала значительное влияние на разницу aPTT-FS и aPTT-LA. Эти данные демонстрируют сопоставимость данных, оцененных с диапазонами концентрации цитрата. Исследование было опубликовано в августе 2018 года в журнале "Архивы патологии и лабораторной медицины" (Archives of Pathology & Laboratory Medicine).
