Низкие уровни глюкозы в крови у диабетиков связаны с риском повторной госпитализации

Такие методы лечения могут значительно повысить уровень инсулина, что, в свою очередь, может привести к слишком низкому уровня глюкозы в крови. Гипогликемия может быть опасной и, в зависимости от тяжести состояния, характеризуется различными симптомами, включая головокружение, спутанность сознания, тревогу, припадки или потерю сознания.

Эндокринологи и их коллеги из Медицинского центра департамента по делам ветеранов Балтимора (Балтимор, Мэриленд, США) исследовали, подвергаются ли пациенты с диабетом и низкими значениями глюкозы в течение последнего дня госпитализации повышенному риску повторной госпитализации или смертности. В ходе оценки показателей повторной госпитализации и смертности у более чем 843 000 пациентов было установлено, что низкий уровень глюкозы в крови также связан с более высокими комбинированными показателями 30-дневной повторной госпитализации и смертности и более высокими показателями смертности после выписки в течение 30, 90 и 180 дней.

Исследователи установили, что относительные риски (ОР) постепенно увеличились во всех пяти вариантах исходов (показатель 30-дневной повторной госпитализации, 30-, 90- и 180-дневной смертности, сочетание 30-дневной повторной госпитализации/смертности). При этом минимальная концентрация глюкозы постепенно снижалась и находилась в промежутке от менее 90 до 99 мг/дл, по сравнению со 100 до 109 мг/дл (ОР 30-дневной повторной госпитализации: 1.01 – 1,45; ОР 30-дневной повторной госпитализации/смертности: 1.01 – 1,71; ОР 30-дневной смертности: 0.99 – 5,82; ОР 90-дневной смертности: 1,01 – 2.40; ОР 180-дневной смертности: 1.03 – 1,91). Показатели 30-дневной повторной госпитализации, 30-, 90- и 180-дневной смертности после выписки и комбинированный показатель 30-дневной повторной госпитализации/смертности были выше для пациентов с диабетом с уровнем глюкозы <92,9 мг/дл, <45,2 мг/дл, 65,8 мг/дл и 67,3 мг/дл и <87,2 мг/дл соответственно.

Авторы пришли к выводу, что у больных сахарным диабетом, имевших гипогликемию или близкие к норме значения глюкозы в течение последних суток госпитализации, отмечались более высокие показатели 30-дневной повторной госпитализации и смертности после выписки. Исследование было опубликовано 1 мая 2019 года в журнале Clinical Endocrinology & Metabolism.
