 糖尿病肾病(DN)是糖尿病的一种主要的微血管并发症,它是终末期肾病的首要病因,发炎对于糖尿病肾病的形成有着某些重要作用。

白细胞介素19 (IL-19)是新发现的一种细胞因子,属于白细胞介素10 (IL-10)家族。IL-19细胞因子在许多发炎过程中有不可或缺的功能,还能激发内皮细胞的促血管生成潜能。

位于镇江的江苏大学临床医学院的科学家2015年7月至12月期间挑选了200名2型糖尿病(T2DM)患者(109名男性,91名女性,年龄为60 ± 10.3岁)。根据尿白蛋白分泌率(UAE)把患者分成三组:102名T2DM患者是正常白蛋白尿(UAE < 30毫克/24小时),72名T2DM患者是微量白蛋白尿(UAE: 30至300毫克/24小时),26名T2DM患者是巨量白蛋白尿(UAE ≥300毫克/24小时)。招募了50名健康献血者作为对照组。

评估所有受试者的IL-19、高灵敏度C反应蛋白(Hs-CRP)、胱抑素C、尿白蛋白分泌率(UAE)和糖化血红蛋白A1C (HbA1c)。用美国明尼苏达州明尼阿波利斯市研发系统公司的酶联免疫吸附测定(ELISA)试剂盒确定IL-19。用德国慕尼黑西门子公司的BN Prospec执行粒子增强型免疫比浊测量,评估尿白蛋白和Hs-CRP的浓度。用日本东京Tosoh公司的G8-90SL执行液相色谱法,测量HbA1c。通过酶方法测量空腹血浆葡萄糖(FPG)、总胆固醇(TC)、高密度脂蛋白(HDL-C)和LDL胆固醇(LDL-C)。

科研小组发现,糖尿病肾病患者血清中的IL-19水平远远高于对照者。IL-19水平与Hs-CRP、胱抑素C、UAE和HbA1c显著正相关。多变量逻辑回归分析发现,IL-19水平(95%置信区间,20.1到62.9)这一个参数与糖尿病肾病显著正相关,甚至针对年龄、性别、高血压和血脂做出调整后。微量白蛋白尿/巨量白蛋白尿糖尿病组的HbA1c远远高于正常白蛋白尿组和对照组。

该研究的论文发表于2017年2月15日的《中心生物医学》杂志《肾病学》分册(BMC Nephrology)。作者总结说,糖尿病肾病患者的IL-19水平显著升高,且与Hs-CRP、胱抑素C、UAE和HbA1c相关。结果表明IL-19不仅对肾小球的病理生理有促炎作用,还对加速肾小球损伤起着重要作用,它使我们更深入地理解IL-19,为糖尿病肾病的治疗和预防找到可能有效的消炎疗法。
